My Healthy Baby

My Healthy Baby is connecting Indiana families to pregnancy and infancy support. The goal of this program is to identify women early in their pregnancies and connect them with a home visitor who provides personalized guidance and support to a woman during her pregnancy and at least the first six to 12 months after her baby’s birth. To begin that effort, the program is working to connect pregnant women who are covered by Medicaid in high-risk areas to services in their communities

Improves communities by expanding access to exceptional health care. By following a Patient-Centered Medical Home model, HealthLinc can provide care that is coordinated, comprehensive and accessible in order to achieve improved health care outcomes and satisfaction for you and your family.
