COVID-19 Resource Hub
The goal of this program is to identify women early in their pregnancies and connect them with an OB navigator – a home visitor who provides personalized guidance and support to a woman during her pregnancy and at least the first six to 12 months after her baby’s birth. To begin that effort, the program is working to connect pregnant women who are covered by Medicaid in our highest-risk areas to services in their communities. This program went live in La Porte County in June 2020.

Healthcare Foundation of La Porte
Dedicated to empowering our residents to live healthy and well in and around La Porte to become one of Indiana’s top 10 healthiest communities by 2030. TEN2030 is funded and maintained by the Healthcare Foundation of La Porte (HFL) to empower residents to live healthy and well in and around La Porte. This site provides health data specific to La Porte County, best practices, and information about local resources within our community. TEN2030 will highlight assets within our community while identifying the gaps that need to be addressed in order for La Porte to become one of the top 10 healthiest communities in Indiana by 2030.
National Domestic Violence
Operating around the clock, seven days a week, confidential and free of cost, the National Domestic Violence Hotline provides lifesaving tools and immediate support to enable victims to find safety and live lives free of abuse. Callers to The Hotline at 1-800-799-SAFE (7233) can expect highly trained, experienced advocates to offer compassionate support, crisis intervention information, educational services and referral services in more than 200 languages.
Indiana Hoosier Healthwise
Hoosier Healthwise is a health insurance program for Indiana children, pregnant women, and low income parents/caretakers.
